Provides support to individuals who have experienced the loss of a loved one. Provides companionship to enable adults to function in their own home. Personal care, meal preparation, or companionship. Aftercare support provided for individuals recently discharged from a hospital or is homebound and in need of care, regardless of age of individual in need. Security- 24 hours, seven days a week care availability. Health Care aides are available to meet health related needs (registered nursing consultation, bedridden client care, etc.) Referrals may be made via a physician, the client, a family member, or other responsible health care provider.
